Required Intake Forms

INTAKE FORMS SHOULD BE COMPLETED AT THE TIME OF BOOKING.

As part of our partnership with Docovia, all new clients are required to complete a Good Faith Exam (GFE) prior to beginning treatment at 608 Wellness & Skin Co.

Failure to complete forms and GFE on time may result in your appointment being rescheduled or canceled.
